Severe Flat Foot Reconstruction with Double Calcaneal Osteotomy and Lateral Column Lengthening
In cases of severe flat foot deformity (PCFD Types 1A to 1D), precise surgical planning and technique are crucial for restoring foot alignment and function. Dr. Pradeep Moonot recently performed a double calcaneal osteotomy, combining Medial Displacement Calcaneal Osteotomy (MDCO) with Lateral Column Lengthening (LCL) to achieve excellent talar head coverage and midfoot realignment.
To reinforce medial arch stability, the procedure was supplemented with a quadrangular ligament construct addressing both the spring and deltoid ligaments, along with FDL (flexor digitorum longus) tendon transfer. These combined interventions allow for robust correction in multi-planar deformities.
Lateral column lengthening plays a pivotal role in such reconstructions—realigning the forefoot and improving load distribution. Dr. Moonot welcomes peer insights on the use of LCL in managing advanced adult-acquired flatfoot deformities.
