Old malunited calcaneum fractures can cause pain, walking difficulties, limping, and restricted ankle movement. Initial treatment involves lifestyle changes and proper footwear, but surgery becomes necessary if these fail. The surgical goal is to restore heel height, length, and reduce heel widening - key problems in such fractures. In this case, old implants were removed, the lateral wall was cleared, and peroneal tendons cleaned. Hindfoot height was restored using an allograft and stabilized with three screws. This approach helps improve function, reduce pain, and restore mobility for better quality of life.
